Prevention / Detection Services
Adult Health: Most services offered at the Health Center are either prevention or detection oriented. The staff does not diagnose, prescribe for or treat any patients except for podiatry care. If an assessment reveals a medical problem requiring treatment, clients will be referred to their physician. Services are scheduled by appointment for residents at the Health Center. Public Health Nurses provide assessment, counseling and health education literature to all residents.

Screenings

The following screenings are available:
  • Blood pressure monitoring and assessment
  • Urine screening
  • Hemoccult kits (low-cost fee applies)
  • Diabetic screening
  • Rapid HIV testing
  • Vision evaluations for residents, refractions for those financially eligible (low-cost fee applies)
  • Mantoux (Tuberculin) screening (low-cost fee applies)
  • Annual blood profile screening (low-cost fee applies)
  • Annual Flu and Pneumonia Vaccination Program
  • Women’s Wellness Program – pregnancy tests, prescription for oral contraceptives, pap tests (low-cost fee applies)
  • Cholesterol, triglyceride screening (low-cost fee applies)

Health Services
Other services include:
  • Childhood Immunizations: Immunizations are administered to school age children 7 to 18 years old based on New Jersey state requirements for income eligible resident.
  • Communicable Diseases: Diseases of public health concern are recorded and investigated by the Nursing Division with assistance from the Environmental Division when indicated. The staff is trained in the statewide epidemiological computer program where communicable disease data is registered. Trends and clusters may be observed and the information utilized.
  • Tuberculosis Control: The Nursing Division is responsible for investigation, assessment, and follow-up of reactive tuberculosis patients. Evaluation and treatment is provided by a consulting pulmonary specialist. If indicated, medications are dispensed free to our clinic patients.
  • Nutrition Information: Residents are assisted with information on how to access the Meals on Wheels program, nutrition site, and/or other nutrition information and needs.

Senior Citizen Services
Residents who are 60 years of age and older may participate in the following services:
  • Nursing Assessment: The nursing program evaluates the total health status of the senior citizen. Included in this assessment are multiscreenings for the detection of hypertension, diabetes, hearing deficits, and urological problems. Health problems are monitored via physician requests.
  • Home Nursing Visits: Nursing assessments and maintenance to home-bound seniors are performed. Referrals are made to appropriate agencies for supportive and supplemental care.  This service is partially funded by the Essex County Division of Senior Services.
  • Vision Screenings: Screenings for visual acuity, tests for glaucoma, cataracts, and refractions are offered to seniors who are financially eligible.
  • Podiatry Care: Evaluation and treatment of foot disorders is performed by a podiatrist for income eligible residents. Medicare assignment is accepted.
  • File of Life: This file will be accessible to emergency medical personnel that states any medications that are taken and / or any other important personal health information. A health history card is placed in a magnetic file holder and put on the front of the refrigerator door.  $1.00 fee is required.
